A small, relationship-driven practice designed to offer longer visits, easier access to your physician, and care that evolves with you over time.
Our aim is to create primary care designed around you—personal, intentional, and centered on lasting relationships. The vision of this practice is to remain small and relationship-driven, allowing me to care for patients over decades and truly know them beyond their diagnoses.
Patients bring a wide range of goals and health needs—from longevity and performance to chronic disease and weight management. I care for all adults, with a thoughtful focus on women's health, recognizing that women's bodies and life stages involve unique medical considerations that benefit from a more individualized approach—while offering equally comprehensive, attentive care across men's health as well.
Your health deserves care that reflects who you are. This is a small, relationship-driven practice designed to offer longer visits, easier access to your physician, and care that evolves with you over time. We take the time to understand your goals, risks, lifestyle, and concerns so your care plan is truly tailored to you. This model of care restores what modern medicine often lacks: time, presence, and a genuine doctor–patient relationship—paired with modern diagnostics, evidence-based prevention, and thoughtful, up-to-date treatment strategies.
Thoughtful medicine. Proactive care. Real partnership.
We provide care that goes beyond treating illness—supporting you at every stage of life, whether for prevention, chronic disease, or new concerns. Our approach rests on three pillars:
Healthy Living, Optimal Aging, and Thoughtful Prevention
We focus on helping you live well today and age well tomorrow. Through personalized screening, lifestyle guidance, and proactive prevention, we target conditions such as coronary artery disease, cancer, diabetes, metabolic disease, hypertension, obesity, osteoporosis, cognitive health, sleep, and women's health—from reproductive care to menopause.
Relationship-Driven Care for Ongoing Conditions
For patients managing chronic conditions, weight goals, or seeking expert women's health guidance, we provide long-term, individualized care. Conditions we manage include diabetes, hypertension, hyperlipidemia, coronary artery disease, thyroid and autoimmune disorders, osteoporosis, gastrointestinal concerns, obesity, and medication management. Care is coordinated with specialists when needed, helping you feel supported, informed, and confident.
Thorough Evaluation When New Concerns Arise
We take the time to listen, assess, and investigate new symptoms with a detailed, evidence-based approach. When specialist care is required, we guide you to leading specialists and coordinate seamlessly, ensuring your care is integrated, comprehensive, and attentive to your unique needs—including women's health and weight management.
Together, these pillars provide care that is personalized, proactive, and deeply relationship driven. You are not just a chart—you are a person, and your story, goals, and health priorities guide every decision.

Dr. Jin-Hwa Simon is a board-certified internal medicine physician with over two decades of experience caring for adults at every stage of health and illness. She completed medical school at Medical University of South Carolina in Charleston, South Carolina, and trained in internal medicine at Cedars-Sinai Medical Center in Los Angeles.
For much of her career, Dr. Simon practiced as a hospitalist, caring for patients at their most vulnerable moments—often when chronic conditions had progressed, complications had developed, and families faced life-altering diagnoses. That work taught her how to recognize subtle warning signs, manage complex problems, and appreciate how much suffering can be prevented with early, thoughtful care.
After years of treating advanced disease, Dr. Simon chose to shift her focus—not away from complexity, but upstream—toward prevention, early detection, and long-term partnership. She wanted the time and continuity to help people stay well, manage chronic conditions, and feel truly known by their physician.
Medicine today can be fast, fragmented, and protocol driven. While guidelines and technology are essential, Dr. Simon believes in restoring the human side of care—listening, understanding context, and treating the whole person rather than just a problem list. Everyone's health story is different, and meaningful care takes time, curiosity, and continuity.
Dr. Simon comes from a background grounded in years of hands-on clinical work and steady commitment to patients—the kind of experience built by showing up day after day in demanding, high-acuity settings. This experience has shaped her clinical judgment, humility, and dedication to thoughtful, evidence-based care.
Her training and experience in hospital medicine inform everything Dr. Simon does in primary care—from careful diagnostics to coordinated specialist care—so that problems are addressed early and care remains proactive and precise.
Dr. Simon is building a practice rooted in continuity and trust—where care unfolds over time, not in fragments. Her goal is to be a steady presence for her patients, offering guidance through both the ordinary and the complex moments of life.
If you're looking for a physician who combines deep clinical experience, careful thinking, and genuine partnership, Dr. Simon would be honored to work with you.
